Hospice of Warren County believes in delivering exceptional end-of-life care with dignity, compassion, and respect. As a part of our team, you'll be a crucial pillar in helping patients and their loved ones through their challenging journey.
As a MEDICAL SOCIAL WORKER at Hospice of Warren County, you will:
· Assess the emotional, psychological, and social needs of patients and their families.
· Collaborate with the interdisciplinary team of doctors, nurses, chaplains, dieticians, councelors to develop a plan of care to address needs related to terminal diagnosis.
· Assist patient, family, and staff in utilizing community resources to meet defined needs.
· Provide counseling, support, and resources to address emotional and practical issues.
· Attend and collaborate with a team of dedicated professionals, including our medical director, nurses, chaplains, bereavement & volunteer coordinators.
· Facilitate communication between patients, their families, and the healthcare team.
- Maintain detailed records and documentation.
- Assess patient/family ability to meet financial obligations and offer appropriate resources and activate as indicated.
- Evaluate patient insurance coverage and options related to hospice benefit.
- Assist in discharge planning from hospice service when care is no longer appropriate.
- Baccalaureate candidates will be provided monthly supervision by a MSW.
QUALIFCATIONS: To be an eligible candidate, you should have:
· A Bachelor's or Master’s degree in social work -or-- related field.
-1 year social work-related experience in healthcare setting
· proof of Licensure or related field -degree- as required by Pennsylvania state regulations.
· A compassionate and empathetic personality.
· Excellent communication and interpersonal skills.
· The ability to work effectively in a team and independently.

Pay: $27.00 - $31.00 per hour
