As a Medical Social Worker, you’ll support patients and families as they navigate health changes, care decisions, and transitions between care settings. You’ll assess psychosocial needs, provide counseling, and coordinate practical support with patients, caregivers, and clinical teams. Responsibilities
- Complete intake, patient assessments, and diagnostic evaluations; document findings in medical records and electronic medical record (EMR) systems.
- Provide individual and group counseling, including child and family counseling, addiction counseling, behavioral health support, crisis intervention, and appropriate psychotherapy.
- Coordinate case management, discharge planning, hospice and end-of-life care, and community resources in collaboration with interprofessional care teams.
- Maintain accurate medical documentation and support data collection or clinical research as assigned.
- Follow social work practice standards, clinical ethical guidelines, and the Health Insurance Portability and Accountability Act (HIPAA).
Experience
- Social work practice and clinical counseling experience; familiarity with motivational interviewing, cognitive behavioral therapy, behavioral therapy, or applied behavior analysis is valuable.
- Experience in medical social work, patient care, hospital, acute, inpatient, outpatient, clinic, hospice, palliative, home and community, post-acute, assisted living, or nursing home settings is relevant.
- Knowledge of geriatric and senior care, dementia and Alzheimer’s care, pediatrics, developmental disabilities, autism, special education, post-traumatic stress disorder (PTSD), and culturally responsive care is welcome.
- Familiarity with Medicare, medical terminology, medical records, and International Classification of Diseases (ICD) coding, including ICD-9 and ICD-10, or Current Procedural Terminology (CPT) coding is helpful.
- Basic life support (BLS) or cardiopulmonary resuscitation (CPR) knowledge is valued.
Pay: $25.00 - $30.00 per hour
Expected hours: No more than 40.0 per week
