We're looking for a compassionate, experienced Medical Social Worker to join our team serving patients across Tarrant County.
What You'll Do
- Assess patients' psychosocial, financial, environmental, and community needs — and connect them with the resources that help.
- Participate in interdisciplinary team meetings and collaborate on individualized care plans.
- Support patients and families through complex emotional and practical challenges with empathy and skill.
- Maintain working knowledge of public and private eligibility standards to advocate effectively for patients.
- Document care accurately and in compliance with professional and regulatory standards.
- Promote Arcy's philosophy of compassionate, patient-centered end-of-life care.
What You Bring
- Current, valid Texas social work license.
- Prior hospice, home health, or senior care experience — strongly preferred.
- A calm, empathetic presence in high-emotion situations.
- Self-directed and dependable — you manage your caseload with minimal supervision.
- Flexible and adaptable — hospice social work rarely follows a script.
- Valid Texas driver's license, reliable transportation, and auto insurance.
- Must be 21 years of age or older.
