HealthFirst Family Care Center, a Certified Great Place to Work and wonderful place to make a difference, is seeking a Psychiatric Mental Health Nurse Practitioner to join our team of mission-minded healthcare professionals.
The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides comprehensive psychiatric evaluation, diagnosis, medication management, and Medication-Assisted Treatment (MAT) services to patients of HealthFirst in accordance with professional standards, evidence-based practices, and quality performance expectations.
The PMHNP works collaboratively within an integrated care model alongside HealthFirst’s Behavioral Health, Primary Care, and Substance Use Disorder (SUD) teams to support patients experiencing mental health and co-occurring substance use disorders. This role includes assessing, prescribing, and managing psychiatric and MAT medications, supporting recovery-oriented care, and promoting whole-person wellness for adult and youth patients.
DUTIES and RESPONSIBILITIES:
- Provide psychiatric services to clients in a variety of clinical settings during scheduled hours. The scope of services includes psychiatric evaluations and medication management services.
- Assess, diagnose, and treat adults, adolescents, and children experiencing symptoms of behavioral health disorders.
- Examine or order laboratory or diagnostic tests for clients to provide information on general physical condition and mental disorders.
- Design/participate in the development of individualized treatment plans, incorporating best and leading practices into treatment interventions.
- Responsible for timely and accurate completion of medical records, including assessment, treatment plan, progress notes, outcome measures, and billing entries.
- Responsible for making independent decisions based on protocols developed utilizing national guidelines for treatment interventions.
- Collaborate with counselors, social workers, or other treatment team members to coordinate client care.
- Prescribe, recommend, or discontinue drugs and treatments to treat mental, emotional, or behavioral disorders in accordance with the Collaborative Practice Agreement and the state’s Nurse Practice Act.
- Collaborate with guardians, relatives, and significant others on client’s conditions and treatment when clinically appropriate.
- Teach, counsel, and provide emotional support, as it is an integral part of the provider’s role in providing the continuum of care for HealthFirst’s Clients.
- Assume responsibility for ongoing health maintenance by initiating pro-active health care needs of patients to carry out their prescribed medical regime and utilize resources to maintain an optimal level of health and wellness.
- Participate in Quality Assurance and Performance Improvement activities.
- Maintain patient care and practice in accordance with federal, state, and agency guidelines.
- Establish, implement, and evaluate research and standards of nursing practice to assure quality care.
- Actively participate in all agency and department education, training, and in-services to confirm understanding of new information or changes to established clinical policies and procedures.

EDUCATION/EXPERIENCE:
Licensed as a Psychiatric Mental Health Nurse Practitioner in the State of New Hampshire. Graduate of a master’s degree program in Nursing with a focus in Psychiatric Nursing. Experience treating patients with co-occurring mental health and substance use disorders required. DEA registration and ability to obtain/maintain all required prescribing credentials required.
