Generation Clinical Partners (GCP) is seeking a versatile, compassionate, and autonomous Psychiatric-Mental Health Nurse Practitioner (PMHNP) to join our behavioral health team. This unique role bridges two critical areas of care: providing acute psychiatric services within our behavioral health hospital and delivering consultative geropsychiatric care across designated assisted living / long-term care facilities.
The ideal candidate thrives in dynamic environments, effortlessly shifting from acute stabilization in a hospital setting to managing complex, age-related psychiatric conditions in assisted living and long-term care facilities. Key Responsibilities
1. Behavioral Health Hospital (Acute Care)
- Assessments & Admissions: Conduct comprehensive psychiatric evaluations, mental status examinations, and risk assessments for newly admitted acute patients.
- Treatment Planning: Formulate, implement, and monitor evidence-based psychopharmacological and therapeutic treatment plans.
- Crisis Intervention: Provide immediate psychiatric intervention and stabilization for patients experiencing acute psychosis, severe depression, mania, or substance withdrawal.
- Multidisciplinary Collaboration: Partner with psychiatrists, social workers, therapists, and floor nurses to ensure cohesive, patient-centered care and safe discharge planning.
2. Assisted Living & Long-Term Care (Consultative Care)
- On-Site Consultations: Travel to contracted nursing homes to evaluate residents experiencing cognitive decline, behavioral disturbances, mood disorders, or severe chronic mental illness.
- Geropsychiatric Management: Specialize in managing the Behavioral and Psychological Symptoms of Dementia (BPSD), depression, anxiety, and late-onset psychosis in geriatric populations.
- Gently Optimizing Medications: Conduct regular medication reviews with a strict focus on gradual dose reductions (GDRs) and minimizing polypharmacy in accordance with CMS regulations.
- Staff & Family Support: Provide guidance and education to nursing home staff and families regarding behavioral management strategies and expectations of psychiatric treatments.
Qualifications & Requirements
- Education: Master's degree from an accredited Nurse Practitioner program with a specialization in Psychiatric-Mental Health (PMHNP).
- Licensure: Active, unrestricted Advanced Practice Registered Nurse (APRN) license and PMHNP Board Certification (ANCC) in the state of practice.
- Prescriptive Authority: Current DEA registration and state controlled-substance license.
- Experience: Minimum of 2 years of experience as a PMHNP is preferred, with a strong background in either inpatient psychiatry or geropsychiatry.
- Skills: Exceptional clinical autonomy, strong communication skills, a team player in the care team, proficiency with Electronic Health Records (EHR), and a deep understanding of CMS guidelines on compliance and regulatory standards
