Schedule: FT, Monday-Friday, 8a-5p
ROLE AND RESPONSIBILITIES
How you'll make an impact in this role:
- Reduce Readmissions: Proactively monitor high-risk patients post-discharge, ensuring smooth care transitions to prevent unnecessary hospital returns.
- Bridge Communication Gaps: Act as the central point of contact, connecting patients, families, primary care providers, and specialists to ensure seamless care coordination.
- Remove Barriers to Care: Identify and address social determinants of health—such as transportation needs, financial constraints, and medication access—to ensure steady recovery.
- Empower Patient Self-Management: Educate patients on managing their chronic conditions, medication adherence, and recognizing early warning signs to promote long-term independence.
BASIC QUALIFICATIONS
What minimum qualifications you'll need:
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted required.
- Registered Nurse credentialed from the Oklahoma Board of Nursing required.
- Licensed Registered Nurse obtained prior to hire date or job transfer date required.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
PREFERRED QUALIFICATIONS
What additional requirements you'll need:
- Experience in Emergency Department or Urgent Care strongly preferred.
- Experience in Population Health, Case Management, Primary Care, Urgent Care or hospital bedside nursing.
- Understanding or knowledge of community resources available to patients.
- Must possess effective communication skills to work with patients, families and health care professionals while also providing clinical assessments, patient education and promoting patient safety as they transition from hospital to home.
- Strong ability to work collaboratively with the clinic provider, care managers, specialists and community resources to ensure effective transition of care.
