Job Description Summary:
Navigates patient and family members through the diagnosis, treatment, and follow-up of the patient throughout the continuum of care ensuring smooth transitions between care settings. Collaborates with a multi-disciplinary team to support patients and families in addressing the following dimensions of care: education, psychosocial needs, advocacy, and continuity of care.

We’re Looking For:
- Registered Nurse license issued by the state in which the team member practices
- Bachelor's Degree in Nursing
- Typically requires 5 years of experience in nursing that includes performing nursing assessments, care plan development, and educating patients
What You’ll Do:
Monday - Friday: 8:00am-4:30pm, no weekends or holidays
- Provides continuity of care by ensuring smooth transitions between care settings. Develops a relationship with patient and their multidisciplinary team to facilitate and/or navigate through subsequent treatment and follow-up to reflect continuity of care.
- Completes a nursing assessment to identify the patient's individualized needs and collaborates with multidisciplinary team and other departments to develop an interdisciplinary plan of care.
- Supports the patient and family by providing education, addressing psychosocial needs, advocating, providing continuity of care and identifying barriers to care.
- Navigates patients and family members throughout the diagnosis, treatment and follow-up of the patient throughout the continuum of care.
- Provides expert clinical advice to the multidisciplinary team in designing collaborative teaching plans/programs and in planning/providing patient/family education and discharge planning including wellness and health maintenance within designated specialty.
- Coordinates timely, seamless, evidenced based care across the care continuum resulting in best patient outcomes.
About This Location:
Our main location is in the Medical Office Center at 17850 S Kedzie Ave., Suite 3100, in Hazel Crest, IL, with additional locations in Orland Park, South Holland, and Frankfort, IL. Our practice includes seven providers, four OB/GYN physicians, one gynecologist, two Advanced Practice Nurses and a dedicated social worker. We provide comprehensive obstetric and gynecologic care to women of all ages. In office procedures include colposcopy, endometrial biopsy, LEEP and hysteroscopy.

Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Maneuver foot pedals on carts or machines.
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
- Must be able to respond quickly to changes in patient and/or unit conditions.
- Physical Abilities Testing may be required.
- Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.
