How to Get Visa Sponsorship in Family Practitioner
Target federally qualified health centers
FQHCs and rural health clinics frequently sponsor J-1 visa waivers through the Conrad 30 program. These underserved-area employers are far more accustomed to sponsoring international physicians than private suburban practices.
Get your board certification before the job search
ABFM certification signals you are fully practice-ready. Employers who hesitate on sponsorship are more willing when licensure and board certification are already complete, reducing their onboarding risk significantly.
Document your clinical training comprehensively
Specialty occupation petitions for physicians require detailed evidence linking your medical degree to the role. Compile transcripts, training certificates, residency completion letters, and clinical rotation records before sponsorship paperwork begins.

Can I work as a Family Practitioner on OPT before completing residency?
No. OPT authorizes employment in your field of study, but independent physician practice requires completing an accredited residency program and obtaining state medical licensure. OPT alone does not substitute for these clinical training requirements. Most F-1 students in medical fields use OPT during residency or fellowship, not as a licensed attending physician.
Which visa sponsorship pathways are most common for Family Practitioners coming off OPT?
The two most common pathways are the H-1B and the J-1 Conrad 30 waiver. Cap-exempt H-1B sponsorship through nonprofit hospitals or university health systems is available year-round without the lottery. The Conrad 30 waiver requires a commitment to practice in a medically underserved area for at least three years and is administered state by state.
Does a medical degree qualify for a STEM OPT extension?
It depends on your specific degree program's CIP code. Some biomedical or clinical science programs qualify for the 24-month STEM OPT extension, but an MD or DO from a U.S. medical school generally does not fall under STEM-designated categories. Confirm your program's designation with your DSO before assuming you have 36 months of OPT.
How do I find Family Practitioner jobs that offer visa sponsorship?
Migrate Mate is built specifically for OPT and visa-sponsored job seekers, making it the most efficient starting point for finding Family Practitioner roles where employers are open to sponsorship. Filtering by sponsorship willingness saves significant time compared to applying broadly and asking about immigration support after the fact.
Can I start a Family Practitioner job before my H-1B is approved?
Yes, if your employer files an H-1B petition with a timely filed receipt before your OPT expires, you may be covered by a cap-gap extension if applicable, or continue working under OPT until the petition is adjudicated. The specifics depend on your OPT expiration date, petition timing, and whether your employer filed premium processing. Consult your DSO and an immigration attorney to confirm your specific situation.
