Midwife Jobs for OPT Students
Midwife roles in the U.S. require state licensure, clinical credentialing, and an employer willing to sponsor OPT work authorization. Most positions sit in hospital systems, birth centers, and federally qualified health centers that routinely file H-1B petitions, making them realistic targets during your 12-month OPT window.

Verify your credentials transfer before applying
Most states require AMCB or NARM certification before issuing a midwifery license. Confirm your foreign credentials are evaluated and your state license is in hand before targeting employers, since many won't start OPT paperwork without active licensure.
Target hospital systems with active H-1B histories
Large hospital networks and academic medical centers file H-1B petitions every year, which signals they have immigration infrastructure in place. Prioritize employers with dedicated HR immigration teams, not independent birth centers that may have never sponsored a foreign national.
Use OFLC Wage Search to anchor your salary ask
Employers must pay the DOL prevailing wage for your role, location, and experience level when sponsoring OPT-to-H-1B transitions. Running an OFLC Wage Search before negotiations shows you understand the floor and prevents low offers that would later fail LCA certification.
Flag E-Verify enrollment when screening employers
STEM OPT extension requires your employer to be enrolled in E-Verify. Midwifery qualifies as a STEM occupation under SOC 29-9099, so confirming E-Verify status at the offer stage protects your ability to extend OPT by 24 months instead of exhausting your 12-month window.
Start H-1B paperwork at least six months before OPT expires
H-1B petitions for the April lottery must be filed months before cap registration opens. Use Migrate Mate to map your OPT end date against the H-1B timeline, so your employer knows exactly when they need to act to keep your status uninterrupted.
Clarify on-call and per-diem arrangements in your offer letter
OPT requires employment that matches your authorized role and hours. Midwifery positions often include irregular on-call shifts or split-site assignments across multiple facilities, which USCIS scrutinizes. Get your offer letter to specify your primary employer of record and expected weekly hours before signing.

Find Midwife JobsFrequently Asked Questions
Does a midwifery role qualify for the STEM OPT extension?
Yes, midwifery is classified under SOC code 29-9099 in O*NET, which falls within a STEM-designated field. That makes you eligible to apply for the 24-month STEM OPT extension after your initial 12-month OPT period, provided your employer is enrolled in E-Verify and your role meets the practical training criteria set by USCIS.
Do I need a U.S. state license before an employer will sponsor my OPT?
Almost always. Midwifery is a licensed profession in every state, and most hospital systems and birth centers won't submit OPT-related paperwork until you hold a valid state credential. If your foreign training requires a credential evaluation before licensure, start that process before graduation so you're not losing OPT time while waiting on paperwork.
How do I find midwifery employers who already sponsor international workers?
DOL LCA disclosure data shows which employers have filed Labor Condition Applications for midwifery and related clinical roles. Migrate Mate surfaces this data so you can filter by employer and role type, giving you a targeted list of hospital networks and health systems that have sponsored international workers before, rather than approaching employers cold.
Can I work at multiple birth centers or hospital locations on OPT?
OPT authorizes you to work for a single employer of record in your authorized field. If your position involves assignments across multiple facilities, your primary employer must be clearly identified and listed as the OPT employer. Informal multi-site arrangements that lack a single sponsoring entity can put your status at risk, so clarify the employment structure in your offer letter before starting.
What happens to my OPT status if my employer can't file an H-1B petition in time?
If your employer misses the H-1B cap registration window and you're on standard 12-month OPT, your work authorization ends when OPT expires. If you've secured the STEM OPT extension, you gain an additional 24 months of runway, which covers the next H-1B registration cycle. Planning your timeline around USCIS's April registration window is critical for midwifery roles where finding a new sponsoring employer mid-year is difficult.
