Nurse Patient Navigator
Paterson, NJ, United States (Hybrid)
JOB DESCRIPTION
Serves as the point of contact for referring physicians, patients/families and caregivers to provide resources and assistance with accessing clinical, supportive care and financial services offered and available in the St. Joseph Healthcare System and community. Coordinates activities of the multidisciplinary team to ensure coordination of services and continuity of care for the patient. Anticipates barriers to care and services and intervenes to ensure the continuity of efficient/timely care. Facilitates transitions of care to assure that patients have appropriate referrals and follow-up appointments. Delivers and coordinates patient education to ensure quality outcomes including the patient's compliance with plan of care.
QUALIFICATIONS
Knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in Nursing, and three to five years of previous work related experience. Licensure required as a Registered Nurse by the State of New Jersey. Successful completion of a department specific orientation and department specific certification of Basic Life Support (BLS) within one year of hire. Knowledge of Microsoft Excel and Word plus proficient data entry skills are necessary. St. Joseph’s Health requires all employee certifications for Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) to be granted through the American Heart Association.

Available for Per Diem Employees and Part-time Employees working under 20 hours per week.
Pay transparency: St. Joseph’s Health provides a salary range to comply with New Jersey Law. The rate of pay for each position will be determined based on a variety of factors including the candidate's relevant experience, qualifications, skills, etc. The salary range does not include incentives, differential pay or other forms of compensation.

St. Joseph’s University Medical Center is an academic tertiary care medical center and state designated trauma center, located on the Paterson campus, regularly accepts referrals of difficult or unusual cases from other hospitals and physicians and performs both complex and routine procedures.
