We Are Hiring:

Location: Chicago, IL

Schedule: Day shift | Full-time
Salary range: $39.75 - 56.11 per hour

What You Will Do:

• Deliver comprehensive direct nursing care on a high-acuity telemetry unit by executing physician orders, implementing evidence-based care plans, and precisely monitoring and documenting acute changes in patient conditions.
• Serve as the primary clinical coordinator across multidisciplinary healthcare teams to streamline communication, manage complex treatment plans, and ensure well-coordinated, high-quality patient care.
• Proactively assess, manage, and coordinate comprehensive patient discharge planning needs from admission to transition, collaborating closely with case management and the healthcare team to optimize outcomes.

What You Will Need:

Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.

Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
