TGH Home Care powered by VNA of Florida provides a full array of home care services for TGH patients, and those in the community needing home care. This office continues the same level of care and experience received at Tampa General Hospital.
What You'll Do
- Completefor Facility based outpatient care, physical therapy evaluations and provides case management for their primary patients.
- Develop individualized treatment plans
- Improve mobility, strength, balance, and function
- Educate patients and caregivers
- Coordinate care with physicians and clinicians
- Maintain clinical documentation
- PT needs to be extremely self motivated and growth minded.
- Excellent customer service.
What You'll Need
- Active Florida PT License
- Strong assessment and treatment skills
- Home health experience preferred
- Ability to work independently
