Optum has an immediate opening for a friendly, patient-focused, and detail-oriented Associate Patient Care Coordinator to join our team. The Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service, and adhere to Lean processes. Supports the teams in meeting financial, clinical, and service goals.
Schedule: This is a 37.5-hour work week.
- Mondays from 9:00 am to 5:00 pm
- Tuesday from 8:00 am to 5:00 pm
- Wednesdays from 9:00 am to 4:00 pm
- Thursdays from 8:00 am to 6:00 pm
- Fridays from 8:00 am to 4:00 pm
The final schedule will be determined by the supervisor upon hire.

Location: 4045 Hempstead Turnpike, Second Floor, Bethpage, NY 11714 (Monday through Wednesday and Friday)

PRIMARY RESPONSIBILITIES:
- Greets patients, family members, and guests with a friendly smile
- Assists patients with signing in and completing registration
- Enters and scans patient's demographic and insurance information into Epic
- Verifies insurance eligibility and benefit information; informs patients of insurance benefits
- Collects co-payments and patient balances according to OPTUM payment guidelines
- Ability to maintain a work queue list - complete any pending actions i.e., referrals, scheduling appointments
- Confirms and schedules appointments
- Provides patient-facing check-in and check-out services across multiple departments, delivering a positive experience while maintaining accuracy and efficiency
- Assists physician and clinical staff by coordinating follow-up care for patients; schedules referral appointments for patients and obtains authorization for diagnostic testing
- Sends patient records to primary care physician
- Assists with other tasks as assigned
- Comply with administrative policies to ensure quality of care
- Demonstrate precision and efficiency in scanning documents and monitoring the fax server, retrieving and/or scanning documents and assigning to the appropriate electronic chart
- All employees are expected to keep abreast on current medical requirements relevant to their position, which includes maintaining patient confidentiality and abiding by all HIPAA and OSHA requirements
- Performs other duties as assigned
