ROLE AND RESPONSIBILITIES
How you'll make an impact in this role:
- Clinical Excellence & Advisory: Assess, review, and optimize complex medication orders to proactively resolve drug-related issues, recommending formulary alternatives and serving as a subject matter expert for interprofessional care teams and clinical leadership.
- Specialized Dispensing & Safety: Oversee the precise compounding, dosing, and dispensing of medications, ensuring rigorous safety standards across specialized patient populations.
- Interdisciplinary Collaboration: Direct-consult with physicians, nurses, and care managers to streamline treatment plans and provide expert drug information to healthcare staff.
- Patient Advocacy & Education: Empower patients through direct medication counseling, helping them safely navigate and understand their therapy regimens.
- Operational Agility: Adapt to dynamic clinical needs by providing flexible coverage across pharmacy units when required.
BASIC QUALIFICATIONS
What minimum qualifications you'll need:
- Licensure/Registration/Licensure: Pharmacist credentialed from the Wisconsin Pharmacy Examining Board obtained prior to hire date or job transfer date required.
- Education: Bachelor's or advanced degree in Pharmacy required.
PREFERRED QUALIFICATIONS
What additional requirements you'll need:
- Doctor of Pharmacy
- Minimum: 2 years of ambulatory care/chronic disease management experience.
- Preferred: 3-5 years of ambulatory care/chronic disease management experience and BCACP.
