We provide outpatient treatment, with cutting edge support for those struggling with substance-use-disorder (SUD). An Arms Acre clinician provides comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; discharge planning; and intervention with client support systems, including social service programs and managed care.
Full Time Schedule:
Hours vary between 7:00am - 7:00pm.
5,000k Sign-on bonus eligible
RESPONSIBILITIES :
- Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures.
- Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames.
- Documents patient progress and own interventions in case record. Need to remain within time frames of regulatory requirements.
- Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director.
- Performs utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities.
- Participates in quality improvement activities facilitating innovation
- Maintains professional competence in their discipline and treatment specialty areas of addiction medicine, through continuing education and supervision.

- Required qualifications are CASAC-T, CASAC certification or LMSW, LMHC
- Experience working specifically with outpatient substance abuse counseling preferred
