Martha's Hands Nurse Care Managers:
Utilize your Nursing Skills as part of a 12-person Care Supervisory Team to:
- Guide a client and their family through their care needs.
- Support/Train our Caregivers in providing direct care to our clients in their homes.
- Partner with home health, hospice and other care professionals to manage their care.
Overview:
Private Pay Clients Only - No Medicare or Insurance Documentation!
Our Nurse Care Managers are compassionate, health care professionals who:
- Conduct Assessments with clients and their families in order to develop a customized Service Plan.
- Coordinate care with other health care professionals including: Home Health, Hospice, Primary Care, etc.
- Orientate and train our Caregivers to provide excellent care.
- Provide clinical oversight of our clients and support to our caregivers.
- Provide direct care to our clients - perform Skilled Nursing tasks and ADL assistance as needed, including Medication Management, Wound Care, Diabetic Care, Tube Feedings, etc.
- Assist families with more detailed Care Management Services as needed.
- Balance both an office and mobile work environment, utilizing technology to enhance care for our clients and equip our Nurse Care Managers to be more
