Overview:
Starting Rate: $24.50/hour
Schedule: Monday - Friday 9am - 5pm with flexibility to meet the needs of the program.
To coordinate, provide and supervise high quality supports to the individuals and providers served by Shared Living program.

Responsibilities:
Responsibilities include, but are not limited to:
- Implementation of all aspects of the program.
- To act as program liaison to local DDS Area Office, Survey & Certification, and other Department personnel as delineated by Program Director.
- Act as liaison with other agencies involved in providing services to the people we support such as day programs.
- Ensure that the program meets and complies with all applicable DDS/Medicaid regulations: overseeing compliance of providers with mandated trainings.
- All responsibilities as identified in DDS regulations re "placement services.”
- Process participant referrals and aspects of the coordination of the admission process.
- Collaborate with the Development Coordinator and participate in recruitment, selection, and evaluation of shared living families.
- Interview families and facilitate the matching and transition of individuals into homes.
- Develop positive relationships with individuals, families and monitor/assess placements minimally on a monthly basis.
- Train staff and families in all facets of their responsibilities.
Qualifications
- BS/BA in related field with 4 years of experience working with individuals with developmental disabilities.
- Ability to communicate effectively verbally and in writing and able to use good judgment.
- Must be able to perform each essential duty satisfactorily.
- Sensitivity to the needs of the individuals we support.
- High energy level, superior interpersonal skills and ability to function in a team atmosphere.
- Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
