SUMMARY OF JOB DESCRIPTION
Position Summary
The Rural Behavioral Health Case Manager focuses on outreach, engagement, and care coordination for individuals and families served through the mobile van program. This role often serves as the first point of contact for clients and plays a vital role in reducing barriers to care such as transportation challenges, stigma, and lack of insurance.
KEY RESPONSIBILITIES
- Conduct proactive outreach and engagement in rural community settings, including schools, food banks, community centers, and local events.
- Serve as a primary point of contact for clients entering services through the mobile van.
- Assist clients with Medicaid eligibility screening, enrollment, and renewal.
- Coordinate referrals to behavioral health treatment, medical care, housing, and other social support services.
- Complete behavioral health screenings for referred clients and/or clients interested in behavioral health services.
- Support clients in scheduling and accessing telehealth services when in-person care is not feasible.
- Maintain consistent follow-up with clients to reduce service drop-off and improve engagement.
- Collaborate closely with therapists and other team members to support integrated care.
- Document outreach, care coordination, and client interactions in Credible EHR.
QUALIFICATIONS
- Bachelor's degree in Social Work, Human Services, Psychology, or a related field (Master's preferred).
- Experience in case management, outreach, or community-based services, preferably in rural or underserved settings.
- Knowledge of Medicaid systems, community resources, and social service navigation.
- Strong engagement, organizational, and communication skills.
- Commitment to trauma-informed, culturally responsive, and client-centered care.
WORK SCHEDULE
This full-time in-person position works a typical schedule of 8 AM - 5 PM, Monday to Friday.
SALARY RANGE
$27.00 to $32.45 an hour, depending on experience, qualifications, and credentials.

ABOUT CONSEJO COUNSELING AND REFERRAL SERVICE
In 1978, Consejo was born out of a visionary work-study project directed by Dr. Gilberto Lucero, a professor at the UW School of Social Work, in which two Latino master level students participated. They identified a critical need for behavioral health services within the Latino community in Seattle, Washington. At that time, there were no Latino-based behavioral health care organizations in WA State. Consejo became a pioneer in establishing the first bilingual and bicultural care organization dedicated to serve the Spanish speaking monolingual community. The mission was to improve mental health care access and break down cultural stigmas. Over four decades later, we continue to lead the way in providing culturally and linguistically competent behavioral health services to the Latinx community, empowering and transforming the lives of our clients.
