As a Full-Time Case Manager (LPN/LVN), you will oversee and manage the delivery of care for residents from pre-admission through post-admission. - Managing care for residents throughout the care continuum, including pre-admission, admission, and post-admission stages.

- Successfully graduated from an approved and certified LVN program
