Sentara Community Care Center Opitz is now hiring a Full-Time Community Healthcare Worker in Woodbridge, VA area!
Hours:
Monday-Friday 8a-430p, Outreach events on Saturday as needed! Some flexibility needed for evening events.
As a Community Healthcare Worker with Sentara, you will work in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. You will engage in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness. You will provide education and behavior modification strategies and provide social support to patients and families. Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.
Bilingual in English and Spanish is preferred. Experience working in a community-based setting preferred. Knowledge of medical terminology preferred.
Qualifications
- Virginia Driver's License - Active/current and in good standing
- High School or equivalent
- Prefer insurance knowledge/experience
- Prefer previous experience in outreach/community service
