General Summary
Provides quality clinical mental health assessments and consultation to providers and supporting clinical care team staff as necessary. They are responsible for triage and management of patients with clinical issues related to their behavioral or psychosocial wellbeing. Additionally, the behaviorist conducts psychosocial assessments, interventions, Transitions of Care (TOC), education, and referrals to inform a holistic treatment plan for patients, in support of Population Health. Essential Functions:
- Conduct thorough biopsychosocial assessments to determine patient needs and appropriate interventions.
- Utilize standardized tools and clinical interviews to assess mental health conditions, substance abuse, and social determinants of health.
- Provide evidence-based individual, group, and family therapy to address mental health and behavioral issues.
- Develop and implement personalized care plans with measurable goals and objectives.
