GENERAL OVERVIEW:
Assesses patient need for diabetes education. Provides individualized diabetes self-management education/training based on assessment. Plans and conducts continuing education programs for professional staff and the community.
ESSENTIAL RESPONSIBILITIES:
- Collects DSME/T program participant assessment data, using a recognized certification database/program in a collaborative and ongoing manner.
- Develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants.
- Collaboratively develops an individualized follow-up plan with each program participant. Evaluates effectiveness of educational services provided by measuring attainment of learning objectives.
- Conducts a follow-up assessment upon completion of DSME/T program services, using outcome measures from the AADE7 Continuum of Outcomes Framework.
- Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records.
- Participates in the development of education curricula and materials.
- Performs other duties as assigned or required.
QUALIFICATIONS
Minimum
- Bachelor’s Degree
- Registered/Licensed Dietitian.
- Certified Diabetes Care and Education Specialist (CDCES) or eligible and must obtain CDCES certification within nine months of hire
- Experience in counseling patients with diabetes and experience in community education.
- Act 34 Criminal Background Clearance Certificate
- Act 33 Child Abuse Clearance Certificate
- Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
Preferred
- Master’s Degree.
- 1-3 years’ experience in Diabetes Education / Health Living Coaching.
- Certified insulin pump trainer (CPT).
