Neurosurgeon Green Card Jobs
Neurosurgeon roles qualify for EB-2 and EB-3 green card sponsorship through the PERM labor certification process, which requires your employer to document that no qualified U.S. worker is available for the position. Academic medical centers, hospital systems, and neurosurgery groups regularly sponsor foreign-trained neurosurgeons for permanent residency.

Get Access To All JobsTips for Finding Green Card Sponsorship in Neurosurgeon
Verify your ECFMG certification before applying
PERM-based sponsorship for neurosurgeons requires proof of U.S. licensure eligibility. Confirm your ECFMG certification and state medical board standing are current before any employer starts the labor certification process, since documentation gaps stall PERM filings.
Target academic medical centers with GME programs
Hospitals with graduate medical education programs sponsor international neurosurgeons more often than community hospitals because they have dedicated immigration counsel and established PERM workflows. Neurosurgery fellowship alumni networks at these institutions often surface open sponsoring positions.
Search green card sponsoring employers on Migrate Mate
Filter neurosurgery roles by employers with active EB-2 and EB-3 filing history on Migrate Mate. This gets you directly to hospital systems and neurosurgery practices that have completed the PERM process before, cutting out employers unfamiliar with physician sponsorship.
Distinguish EB-2 and EB-3 pathways with your employer
Neurosurgeons typically qualify under EB-2 as advanced-degree professionals, but some employers file EB-3 to simplify the PERM recruitment obligations. Ask the employer's immigration counsel which category they intend to file under, since this affects your priority date and overall timeline.
Request the PERM recruitment timeline upfront
DOL requires employers to run specific recruitment steps before filing PERM. Ask your prospective employer when they plan to start recruitment, since the mandatory 30-day job order and 60-day window after posting must complete before DOL submission.
Confirm prevailing wage compliance using the OFLC Wage Search
Your employer must pay at least the DOL prevailing wage for your neurosurgery role and location. Cross-check the offered salary against the OFLC Wage Search before signing an offer, since a wage below the certified level invalidates the PERM filing.

Find Neurosurgeon JobsNeurosurgeon Green Card Sponsorship: Frequently Asked Questions
Do neurosurgeon roles qualify for EB-2 or EB-3 green card sponsorship?
Neurosurgeons typically qualify under EB-2 as advanced-degree professionals, since the role requires a medical degree plus residency and fellowship training. Some employers file under EB-3 if they prefer the PERM recruitment structure for that category. Both pathways lead to permanent residency, but EB-2 priority dates are generally more favorable for most countries of birth.
How does PERM green card sponsorship differ from H-1B for a neurosurgeon?
H-1B is a temporary work visa capped at 85,000 annually and subject to lottery selection, while PERM-based green card sponsorship has no annual cap at the petition level and leads to permanent residency. The PERM process takes longer, often 18 to 36 months from labor certification through I-485 approval, but it provides a permanent immigration outcome rather than a renewable temporary status.
Which employers are most likely to sponsor a neurosurgeon for a green card?
Academic medical centers, university hospital systems, and large neurosurgery group practices are the most consistent green card sponsors for neurosurgeons. These employers have established relationships with immigration counsel, familiarity with the PERM recruitment requirements, and the institutional capacity to manage multi-year sponsorship timelines. You can search employers with verified EB-2 and EB-3 filing history on Migrate Mate.
What is the role of the PERM labor certification in a neurosurgeon's green card process?
PERM is the DOL process that requires your employer to document that no qualified, available U.S. worker applied for the neurosurgeon position despite good-faith recruitment efforts. It must be completed and approved before your employer files the I-140 immigrant petition with USCIS. The PERM stage alone typically takes six to nine months when filed without an audit.
Can a neurosurgeon change employers after PERM is filed?
Once your I-140 is approved and your priority date is current or you've held approved status for 180 days, portability rules under AC21 allow you to move to a similar neurosurgery role without restarting the green card process. Changing employers before I-140 approval generally requires the new employer to begin a fresh PERM filing, resetting your timeline.
