Verify your ECFMG certification before applying
ECFMG certification is required before ECFMG can issue a DS-2019 under the J-1 Physician program. Start this process early, as credential verification and examination requirements add months to your timeline before any program application.
Target programs with active ACGME accreditation
Diagnostic radiology residencies and fellowships must hold ACGME accreditation to function as J-1 host institutions. Filter your program search to ACGME-accredited positions, since unaccredited programs cannot support your DS-2019 sponsorship through ECFMG.
Search Migrate Mate to find J-1 receptive radiology employers
Use Migrate Mate to identify U.S. diagnostic radiology programs and academic medical centers that have historically hosted international physicians. This narrows your search to institutions already familiar with J-1 program requirements and ECFMG coordination.
Understand the two-year home residency requirement early
Most J-1 Physician participants are subject to the two-year home residency requirement under INA Section 212(e). Plan whether you intend to apply for a waiver through a state health agency, the Conrad 30 program, or federal agency before accepting an offer.
Align your training plan with ECFMG program objectives
ECFMG requires a structured training plan that matches your exchange program objectives to specific radiology subspecialties or research activities. Programs that cannot articulate clear training goals risk delays in DS-2019 issuance, which stalls your start date.
Negotiate offer timing around DS-2019 processing windows
Once a program extends an offer, ECFMG needs time to verify credentials and issue your DS-2019. Build at least 90 days between offer acceptance and your intended program start date to avoid gaps caused by documentation or ECFMG processing delays.

Find Diagnostic Radiologist JobsDiagnostic Radiologist J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to diagnostic radiologists?
Diagnostic radiologists almost always enter through the J-1 Physician category, which covers physicians in accredited graduate medical education programs, residencies, and clinical fellowships. ECFMG is the exclusive designated sponsor for this category and issues the DS-2019. Researchers without a clinical training component may qualify under Research Scholar instead, depending on the program structure and host institution.
Who actually sponsors a J-1 visa for a diagnostic radiology position?
The visa sponsor is ECFMG, a U.S. Department of State-designated organization, not the hospital or radiology program where you train. The hospital or academic medical center is your host institution. ECFMG issues your DS-2019, maintains your compliance records, and communicates with the State Department. Your host employer cannot issue a DS-2019 on its own, regardless of size or resources.
How do I find diagnostic radiology programs open to J-1 physicians?
Search Migrate Mate to identify U.S. diagnostic radiology programs and academic medical centers that have hosted J-1 physicians before. Many programs only disclose international candidate eligibility during the interview season, so targeting institutions with prior J-1 experience saves time. Confirm ECFMG eligibility with each program directly before submitting a formal application.
Does the two-year home residency requirement apply to radiology J-1 physicians?
Yes, most J-1 Physician participants are subject to the two-year home residency requirement under INA Section 212(e), which requires returning to your home country for two years after completing your program before changing to most other U.S. immigration statuses. Waivers are available through the Conrad 30 state program, interested government agencies, or a hardship claim, but each path has separate application requirements and timelines.
Can a diagnostic radiologist switch from a J-1 to an H-1B without completing the home residency requirement?
Not directly, unless you obtain a waiver of the two-year home residency requirement first. Without a waiver, you cannot change to H-1B status from within the United States after completing a J-1 Physician program. USCIS will not approve an H-1B change of status or an H-1B visa stamp abroad if the requirement remains on your record. The Conrad 30 program is the most common waiver pathway for physicians who accept underserved area positions.
