Get Access To All JobsTips for Finding J-1 Visa Sponsorship in Patient Care RN
Verify your credentials meet U.S. nursing standards
NCLEX licensure is a hard requirement for most host employers, and CGFNS credential evaluation is often a prerequisite to that exam. Start both processes early since combined timelines can exceed six months before you're placement-ready.
Match your J-1 category to your career stage
The Specialist category fits licensed nurses with a defined training objective, while Research Scholar suits RNs pursuing clinical research roles. Your career stage determines which category your designated sponsor can use to issue the DS-2019.
Search Migrate Mate to find J-1-aligned hospital roles
Filter by role and visa type on Migrate Mate to surface Patient Care RN positions at health systems familiar with hosting J-1 exchange visitors. Starting your search there saves time that generic job boards waste on employers unfamiliar with the exchange visitor framework.
Confirm the host employer has a sponsor relationship
Not every hospital can host a J-1 exchange visitor. Ask prospective employers whether they have an existing relationship with a State Department-designated sponsor organization before investing time in their interview process.
Request a detailed DS-2019 training plan from your sponsor
The DS-2019 must reflect a structured program with clear objectives tied to your Patient Care RN role. Review the training plan before signing, because USCIS and the sponsor will use it to evaluate whether your exchange objective is being met.
Understand the two-year home residency requirement before accepting an offer
Many J-1 nurses are subject to a two-year home-country residency requirement after their program ends, which blocks immediate H-1B or green card transitions. Confirm your country's waiver eligibility with DOL and your designated sponsor during offer negotiations.

Find Patient Care RN JobsPatient Care RN J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to Patient Care RN positions?
Most Patient Care RN placements fall under the Specialist category, which covers professionals with specialized skills and a defined U.S. training objective. Nurses pursuing clinical research alongside patient care may qualify under Research Scholar instead. Your career history and the host employer's program structure determine which category your designated sponsor can support when issuing the DS-2019.
Who actually sponsors my J-1 visa - the hospital or a sponsor organization?
The hospital or health system is your host employer, not your visa sponsor. Your J-1 visa sponsor is a U.S. Department of State-designated organization, such as CIEE or Cultural Vistas, that issues the DS-2019 form, signs your training plan, and monitors your exchange program for compliance. The host employer provides the clinical placement and training environment.
How do I find Patient Care RN roles where employers are already familiar with J-1 hosting?
Use Migrate Mate to filter Patient Care RN positions by visa type, which surfaces roles at health systems experienced with the J-1 exchange visitor framework. Many hospitals are open to hosting international nurses but haven't formalized a sponsor relationship yet, so confirming that relationship early in the interview process saves significant time on both sides.
Does the two-year home residency requirement affect Patient Care RNs?
Many J-1 nurses are subject to a two-year home-country physical presence requirement after their program ends, which prevents immediate changes of status to H-1B or immigrant visa categories. Whether it applies depends on your nationality and funding source. DOL administers Conrad 30 waivers for healthcare workers in underserved areas, which can eliminate this requirement under specific conditions.
Do I need NCLEX licensure before a host employer will consider me for a J-1 Patient Care RN role?
Most U.S. hospitals require active state RN licensure, which means passing the NCLEX, before they'll extend a formal offer to a J-1 candidate. International nurses typically complete CGFNS credential evaluation before sitting for NCLEX. Plan for a combined six-month or longer credentialing timeline before you're ready for clinical placement interviews with host employers.
