Verify your nursing credentials transfer cleanly
State nursing boards vary on foreign credential equivalency. Confirm your home-country RN license and clinical hours satisfy the host state's requirements before engaging employers, since gaps in credential recognition can delay or block your DS-2019 issuance entirely.
Distinguish Trainee from Specialist program eligibility
The J-1 visa Trainee category fits nurses within five years of graduation or last employment in their field. If you have deeper experience in case management specifically, the Specialist category may apply and carries different training plan requirements with your designated sponsor.
Target host employers with established J-1 infrastructure
Use Migrate Mate to identify U.S. healthcare employers and insurers that have hosted J-1 exchange visitors in clinical or case management roles, so you're approaching organizations already familiar with DS-2019 obligations and training plan compliance.
Build a training plan before your offer letter
Your designated sponsor requires a detailed training plan outlining skills you'll develop as a Field Nurse Case Manager host. Draft this document early, aligned to OFLC-recognized competencies for nursing case management, so employers see a ready-to-sign plan rather than a blank requirement.
Check whether your home country triggers the two-year rule
Many nurses from countries on the DOS exchange visitor skills list face a two-year home residency requirement after J-1 status. Confirm your country's status with USCIS before accepting a host offer, since this requirement affects whether you can change status or extend in the U.S.
Align your host offer to your program's start window
Designated sponsors can only issue a DS-2019 once a host employer submits a signed agreement. Coordinate your offer timeline so the sponsor's processing window, typically two to four weeks, doesn't push your program start date past your planned arrival or credential activation date.
Field Nurse Case Manager J-1 Visa: Frequently Asked Questions
Which J-1 program category fits a Field Nurse Case Manager role?
Most internationally trained nurses pursuing case management positions in the U.S. qualify under the J-1 Trainee category if they are within five years of their nursing degree or most recent nursing employment. Nurses with specialized experience specifically in case management and discharge planning may qualify under the Specialist category instead. Your designated sponsor, not your host employer, makes the final determination based on your background documentation.
Who actually sponsors the J-1 visa for this role, the hospital or a separate organization?
The J-1 visa sponsor is always a U.S. Department of State-designated organization, such as AIPT, Cultural Vistas, or CIEE, not the healthcare system or insurer hiring you. That employer is your host organization. The designated sponsor issues the DS-2019 form, monitors your training plan compliance, and serves as your official point of contact with the State Department throughout your program.
How do I find U.S. employers willing to host a J-1 Field Nurse Case Manager?
Migrate Mate lets you search for U.S. healthcare employers and managed care organizations that have experience hosting international clinical professionals under J-1 programs. Targeting employers with existing J-1 infrastructure significantly reduces the time spent educating HR teams about DS-2019 requirements, training plan obligations, and program duration rules before you receive a formal host agreement.
Does the two-year home residency requirement apply to nurses from most countries?
It depends on whether your home country appears on the DOS Exchange Visitor Skills List and whether your J-1 program is government-funded. Nurses from many countries in Asia, Africa, and Latin America are commonly subject to the requirement, which means you must return home for two years before changing to H-1B visa or green card status. Confirm your country's status through USCIS before accepting a host offer.
What documents does a Field Nurse Case Manager need before a designated sponsor can issue a DS-2019?
Sponsors typically require a signed host organization agreement, a detailed training plan specifying the case management competencies you will develop, proof of your nursing credentials and license, evidence of English proficiency, and financial support documentation. Your host employer must also agree in writing to provide adequate supervision and resources for the training objectives outlined in the plan.
