Compensation: $24–$27 per hour
To Stand Out: Email Tom Lecy at tom.lecy@hhcare.net with a brief overview of your background and why you're interested in joining Twin Cities Physicians.

Twin Cities Physicians is seeking an Office Triage LPN to support our providers and more than 4,500 seniors receiving primary care services throughout over 200 senior living communities across Minnesota. This role is ideal for someone who enjoys being the clinical hub behind the scenes, ensuring patients receive timely, coordinated, and compassionate care.

Why You'll Love This Role:
Monday–Friday schedule with no nights, weekends, or holidays
Stable office environment with a collaborative team
Continue using your clinical judgment and nursing knowledge every day
Meaningful, patient-centered work without direct bedside responsibilities
Opportunity to support a rapidly growing geriatric primary care practice
Learn from experienced physicians, nurse practitioners, physician assistants, and support staff
Benefits:
Medical, Dental, HSA, Life Insurance
Short- and Long-Term Disability
401(k) with employer match
Paid Time Off (PTO)
Your Day-to-Day Responsibilities:
Triage incoming phone calls from patients, family members, and senior living staff
Collaborate with providers and internal support teams to ensure timely care
Process provider orders, standing orders, and clinical updates
Review and reconcile medications
Prepare patient charts and maintain EMR documentation
Respond to faxes, refill requests, and incoming clinical communications
Assist with care coordination and ensure timely follow-up on patient needs
Serve as a key communication link between providers, patients, families, and community staff
You're a Great Fit If You:
Hold an active Minnesota LPN license
Have previous experience in primary care, geriatrics, long-term care, or senior living
Possess strong communication and telephone triage skills
Are comfortable spending the majority of the day on the computer and on the phone
Have excellent attention to detail and organizational skills
Can prioritize multiple tasks in a fast-paced environment
Have experience with EMR systems; PCC and Eldermark experience are a plus
Enjoy supporting patient care behind the scenes rather than in a direct patient-facing role
Value teamwork and providing exceptional service

Licensed Practical Nurse Jobs in Minneapolis: Frequently Asked Questions
How do I get a licensed practical nurse job in Minneapolis?
The strongest entry points in Minneapolis are long-term care facilities along the I-394 corridor, home health agencies serving North and Northeast Minneapolis, and the network of federally qualified health centers in Phillips and Cedar-Riverside. Candidates with experience in geriatric care, wound care, or IV therapy stand out locally. Applying directly to health systems with multiple Minneapolis campuses gives you the widest access to open shifts and unit variety.
Which companies hire licensed practical nurses in Minneapolis?
Employers hiring licensed practical nurses in Minneapolis right now include Pediatric Home Service, Fairview Health Services, and Ebenezer, based on current listings on Migrate Mate as of July 2026. Minneapolis draws a mix of large integrated health systems, regional long-term care chains, and independent home health agencies, all of which hire licensed practical nurses on a rolling basis throughout the year.
Are there remote licensed practical nurse jobs in Minneapolis?
Yes, though remote work is limited for licensed practical nurses because most duties require direct patient contact in clinical or residential settings. About 11% of licensed practical nurse openings tied to Minneapolis are remote or hybrid as of July 2026, concentrated in care coordination, utilization review, and telephone triage roles. Minneapolis-area managed care organizations and health plan administrators are the most consistent source of those remote positions.
How can I get a licensed practical nurse job in Minneapolis with little or no experience?
The most realistic entry path in Minneapolis is through a long-term care or assisted living facility, where new graduates are regularly onboarded with structured mentorship. Minneapolis also has several home health agencies that accept recent completers of accredited LPN programs and pair them with experienced RN supervisors for initial visits. Volunteering or completing clinical rotations at Hennepin Healthcare or community health centers in Phillips strengthens a local application considerably.
Which industries hire the most licensed practical nurses in Minneapolis?
Minneapolis licensed practical nurse roles concentrate in Healthcare & Medical Services, based on current listings on Migrate Mate as of July 2026. Minneapolis's density of senior living communities, its network of federally qualified health centers, and the outpatient clinic expansion driven by major regional health systems all sustain consistent demand for licensed practical nurses across those sectors.
