Overview

Acme Health Network is seeking a compassionate, skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric medication management and psychotherapy to older adults in long-term care settings, including skilled nursing facilities, nursing homes, and assisted living communities. In this role, you will deliver person-centered behavioral health care to a population with complex medical and psychiatric needs, working alongside facility staff, attending physicians, and our interdisciplinary team to improve residents' quality of life. Responsibilities

• Conduct comprehensive psychiatric evaluations and follow-up assessments for residents in skilled nursing and assisted living facilities.
• Provide psychiatric medication management, including prescribing, monitoring, and adjusting psychotropic medications with attention to geriatric considerations and gradual dose reduction (GDR) requirements.
• Deliver evidence-based psychotherapy (individual and supportive therapy) tailored to older adults, including those with dementia, depression, anxiety, and serious mental illness.
• Collaborate with facility nursing staff, attending physicians, social workers, and families to develop and coordinate individualized care plans
• Complete timely, accurate, and compliant clinical documentation in the electronic health record in accordance with CMS regulations and payer requirements.
• Participate in facility care plan meetings and provide behavioral health guidance and education to facility staff.
• Maintain a consistent caseload and visit schedule across assigned facilities, ensuring continuity of care.

Qualifications

• Master's or Doctoral degree from an accredited Psychiatric Mental Health Nurse Practitioner program.
• Active, unrestricted NP license in [State] and current PMHNP-BC board certification (ANCC).
• Active DEA registration (or ability to obtain) and state controlled substance registration where required.
• Experience in geriatric psychiatry or long-term care strongly preferred; new graduates with relevant clinical rotations are encouraged to apply.
• Strong clinical judgment, communication skills, and ability to work independently across multiple sites.
• Reliable transportation and willingness to travel to assigned facilities.
