Job Summary
We are seeking a dedicated and compassionate Qualified Mental Health Professional to join our dynamic team. Duties
- A Qualified Mental Health Professional (QMHP) provides direct, collaborative behavioral health services, including intake, psychosocial skills development, crisis de-escalation, and care coordination. The role requires a minimum of a bachelor's degree in a human services field and state-specific credentialing or registration. Responsibilities
- Assessment & Intake: Conduct client screenings, gather mental/physical health histories, and write psychosocial histories.
- Intervention & Treatment: Implement assigned interventions, initiate crisis de-escalation, and develop/update individual care plans.
- Skill Building: Provide client education and psychosocial skills development to help individuals reach stability.
- Care Coordination: Act as a case manager by providing referrals, coordinating with other healthcare providers, and maintaining strict clinical documentation
- Maintain records and documentation Daily Progress Notes BIRP Format, ISP's, Safety Plans, Discharge Summaries, Quarterlies, etc.
