Palliative Care Nurse Practitioner – San Diego, CA
Supportive Care Specialist Group is hiring a full-time, in-person Palliative Care Nurse Practitioner in San Diego. Monday through Friday day shift, with rotating weekend coverage. Hospital and LTAC settings.
About the Role
Supportive Care Specialist Group is looking for a compassionate, experienced Palliative Care Nurse Practitioner to care for adult and geriatric patients with serious illness in San Diego hospitals and Long-Term Acute Care (LTAC) facilities. Work with a high degree of clinical independence as part of a collaborative, multidisciplinary team, helping patients and families navigate important healthcare decisions.
What You’ll Do
- Round daily at hospital and LTAC facilities
- Provide palliative care consults and manage complex symptoms
- Lead goals-of-care conversations with patients and families
- Collaborate with attending physicians, nursing staff, and interdisciplinary teams
- Develop individualized care plans
- Complete timely, accurate clinical documentation
Required Qualifications
- Active California Nurse Practitioner license
- 2+ years of inpatient hospital experience
- 2+ years of primary care experience
- Excellent communication skills and a compassionate bedside manner
Preferred Qualifications
- Experience in palliative care, hospice, or geriatrics
- Bilingual in Spanish
Schedule
- Monday to Friday, day shift
- Rotating weekend coverage
- On-site rounding at hospital and LTAC facilities
