- Evaluate patients' functional status, including mobility, strength, coordination, communication, and ability to perform daily activities
- Assess home environments to identify safety risks and recommend modifications or adaptive equipment
- Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life
- Provide therapeutic interventions to restore function, including retraining in daily living activities
- Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate
- Support patients returning to work through functional and work capacity evaluations
- Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs
- Collaborate with the care team to coordinate services and ensure seamless, patient-centered care
- Participate in discharge planning to support successful transitions and continued progress
- Document patient care accurately and timely within the electronic medical record (EMR)
- Communicate plans of care with physicians and interdisciplinary team members
- Participate in case conferences, team meetings, and ongoing clinical collaboration
- Maintain strong, professional relationships with patients, families, and care team members
- Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations and company policy
- When therapy is the sole skilled service, provide oversight of home health aide care as required
- Strong clinical judgment with a patient-first mindset
- Ability to build trust and rapport with patients and families in a home setting
- Excellent communication, organization, and time management skills

- Graduate of an accredited Occupational Therapy program
- Current registration with the National Board for Certification in Occupational Therapy (NBCOT)
- Current state licensure as an Occupational Therapist
- Minimum of one (1) year of clinical experience preferred (home health experience a plus)
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
