OPT Hospitalist Jobs
Hospitalist roles are among the most OPT-friendly physician positions in the U.S. because hospitals operate year-round and routinely sponsor H-1B visas for clinical staff. STEM OPT does not apply to medicine, so you have 12 months of standard OPT to secure a position and begin the H-1B sponsorship process.

Get Access To All JobsTips for Finding OPT Sponsorship in Hospitalist
Target large hospital systems first
Large academic medical centers and hospital networks like HCA Healthcare and Tenet Health have established immigration pipelines and legal teams experienced with H-1B sponsorship for hospitalists. Smaller community hospitals often lack that infrastructure.
Confirm your ECFMG certification is current
Employers will not begin H-1B paperwork until your ECFMG certification is in order. Verify your status before reaching out to recruiters so you can confirm you are sponsorship-ready without any administrative delays holding up the process.
Apply early relative to your OPT end date
The H-1B lottery opens in March for an October 1 start. If your OPT expires before October, you need a job offer and filed petition well in advance. Count backwards from your OPT expiration and set your application timeline accordingly.
Ask about cap-exempt employer status upfront
Hospitals affiliated with universities or nonprofit research institutions may qualify as cap-exempt H-1B employers, meaning no lottery. This is critical for OPT students who miss the regular cap. Confirm affiliation status directly with the recruiter before investing time in the process.
Get your state medical license sorted before interviewing
Most hospitalist employers require an active state license before they will extend an offer or initiate H-1B paperwork. Licensing timelines vary by state, ranging from six weeks to several months, so start that process as early as your program allows.
Negotiate a start date that aligns with OPT status
Be transparent with employers about your OPT expiration date during negotiations. Most hospital HR teams are experienced with this conversation and can structure start dates and filing timelines to keep you continuously authorized to work without gaps.
Hospitalist OPT: Frequently Asked Questions
Can I work as a hospitalist on OPT?
Yes, you can work as a hospitalist during your OPT period as long as the role is directly related to your medical degree. Hospitalist positions qualify because they require the clinical training you received through your MD or DO program. Your OPT authorization is tied to that field, so the role must match your area of study.
Do hospitalist jobs qualify for STEM OPT extension?
No. Medicine is not a STEM-designated field under the Department of Homeland Security's STEM OPT extension program. Physicians on F-1 OPT receive the standard 12-month period only. This makes it especially important to secure H-1B sponsorship quickly, since you have less runway than peers in engineering or data science fields.
Which types of employers sponsor H-1B visas for hospitalists?
Large hospital systems, academic medical centers, and physician staffing groups are the most consistent H-1B sponsors for hospitalists. Nonprofit and university-affiliated hospitals may qualify as cap-exempt, which removes the lottery requirement entirely. You can browse hospitalist positions from sponsoring employers on Migrate Mate, which filters for roles open to candidates needing visa sponsorship.
How does the H-1B lottery timeline interact with my OPT expiration?
The H-1B cap lottery runs once per year, with registrations in March and an October 1 start date for selected petitions. If your OPT expires before October 1 and you are selected in the lottery, USCIS can issue a cap-gap extension that bridges the gap and keeps you authorized to work. If your OPT expires before the lottery is even filed, you may fall out of status, so timing your job search carefully is essential.
What should I ask a hospitalist employer about visa sponsorship during the interview?
Ask whether the organization has sponsored H-1B visas for physicians before, whether they work with an immigration attorney, and whether their hospital qualifies as cap-exempt. Also confirm the earliest date they can file your H-1B petition relative to your OPT expiration. Employers with experience sponsoring hospitalists will answer these questions directly and without hesitation.
