Schedule: Day Shift | Full Time | Monday-Friday, occasional outreach events
ROLE AND RESPONSIBILITIES
How you'll make an impact in this role:
- Identify and connect with community members to assess their unique needs and guide them toward the most appropriate health and wellness services.
- Advocate for patients by assisting with medical appointments, accompanying them to visits, and simplifying the application process for vital healthcare programs.
- Provide compassionate, informal counseling and education to empower clients with the social support and knowledge they need to manage their health.
- Cultivate strong partnerships with community agencies and internal clinical teams to build a reliable network of resources for the people we serve.
- Document every patient interaction and outcome to ensure no one falls through the cracks and the impact of our services is clearly captured.
BASIC QUALIFICATIONS
What minimum qualifications you'll need:
Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
PREFERRED QUALIFICATIONS
