Position Requirements
Education: Graduate of an accredited nursing program.
Licensure: Current Registered Nurse (RN) license in Minnesota.
Experience: Minimum of two years as an RN; hospice or palliative care experience strongly preferred.
Skills:
Independent work and consultation as needed.
Collaborating with home health aides and facility staff to support patient care.
Crisis intervention, grief support, and advance care planning.
Acceptance of diverse backgrounds, flexibility, and strong interpersonal skills.
Effective time management and attention to detail.
Key Responsibilities
Provide weekend nursing assessments, symptom management, and care consistent with each patient’s individualized plan of care.
Provide timely documentation and communicate changes in patient condition, interventions, and follow-up needs to the RN case manager and hospice team.
Coordinate with hospice providers, pharmacies, facility partners, and caregivers to address patient needs during assigned weekend coverage.
Promote Lake Valley Hospice services with professionalism and care.
Provide scheduled and as-needed nursing visits during assigned weekends, with on-call coverage as scheduled.
Additional Requirements
Valid driver’s license, auto insurance, and reliable transportation.
