Care Coordinator Jobs in Minnesota: Frequently Asked Questions
How do you become a care coordinator in Minnesota?
Most Minnesota care coordinator roles require a bachelor's degree in social work, nursing, or a related health field, with many employers preferring candidates who hold an active Minnesota Board of Social Work license such as an LSW or LGSW. Registered nurses licensed through the Minnesota Board of Nursing also qualify for clinical care coordinator tracks. Earning a Certified Case Manager credential strengthens your application for senior positions.
Which companies hire care coordinators in Minnesota?
Minnesota care coordinator roles are posted by Bluestone Physician Services, Park Dental, and Mayo Clinic and others right now, based on current listings on Migrate Mate as of August 2026. Minnesota's large integrated health systems and nonprofit managed care organizations are among the most consistent sources of care coordinator openings in the state.
Which Minnesota cities have the most care coordinator jobs?
Minneapolis, Saint Paul, and Edina have the most care coordinator openings in Minnesota. The Twin Cities metro drives the largest share of postings because of its concentration of major health systems, managed care plans, and county human services agencies, while Rochester's market is anchored by Mayo Clinic and its affiliated care network.
Are there remote care coordinator jobs in Minnesota?
Yes, but they're less common than in purely desk-based fields because care coordination often involves in-person patient contact, home visits, or on-site team rounds. About 49% of care coordinator openings tied to Minnesota are remote or hybrid as of August 2026, with telephone and telehealth-based utilization management roles being the most likely to allow fully remote work.
How can I get hired as a care coordinator in Minnesota with little or no experience?
The most realistic entry path is applying for a care coordinator associate or patient navigator role at a large Minnesota health system, then building toward full care coordinator status. Employers like Hennepin Healthcare and HealthPartners hire candidates from adjacent roles such as medical assistant, community health worker, or social services case aide. Completing a Minnesota Board of Social Work-approved internship during your bachelor's program and earning a CMC exam prep certificate gives you a measurable edge.
Where can I find and apply to care coordinator jobs in Minnesota?
You can find and apply to care coordinator jobs in Minnesota on Migrate Mate, which lists current Minnesota openings across health systems, managed care organizations, and community health providers. Search the listings for roles that match your experience level and credentials, then apply directly to the ones that fit.
