ABOUT THE ROLE:
As a Care Coordinator, you’ll be the heartbeat of daily operations—ensuring clients receive outstanding care and caregivers feel supported. This role requires someone highly organized, proactive, and team-oriented.
Responsibilities:
- Coordinate and manage daily scheduling of services
- Resolve client and caregiver concerns promptly
- Answer calls with warmth and efficiency
- Maintain accurate care plans and records
- Collaborate with the care team for seamless service
- Participate in on-call rotation as needed
REQUIREMENTS
- 3+ years of home care industry experience
- Experience with AxisCare or similar scheduling software preferred
- Strong multitasking, organizational, and problem-solving skills
- Excellent communication and teamwork abilities
- Comfortable with technology and quick to learn new systems
- Self-starter who goes above and beyond
