BayCare Clinic Cardiology is looking for a Social Worker to join our team in Green Bay, WI. We’re looking for a detail oriented, thoughtful candidate who wants to contribute to a positive experience for all our patients. This role is 20-40 hours a week, between our core hours, Monday-Friday between 8:00 a.m. - 5:00 p.m. No Holidays! No Weekends!
The Clinic:
BayCare Clinic is a successful, dynamic group comprised of 19 specialties. We are a physician-owned entity with joint ownership in Aurora BayCare Medical Center, a 167-bed hospital located in Green Bay, WI, offering us a built-in referral base. Each of our specialty practices have clinical autonomy and are financially sound.

The Job:
- Provides social work services to individuals or groups including:
- Screening
- Psychosocial assessment
- Discharge planning
- Determination of social work goals
- Exercises professional judgment, recognizing deviations from the usual patient/family functioning, anticipating problems and taking measures to maximize coping capabilities.
- Documents activity in keeping with department/program protocols or standards.
- Implements interventions substantiated by assessment as appropriate to the needs of the patient/client system and consistent with available resources. This would include referrals to home care, palliative care, hospice, assisted living, and skilled nursing facilities when appropriate.
- Advocates for patient involvement in the plan of care.
- Initiates and coordinates interventions with the activities of other members of the healthcare team.
- Collaborates with community agencies and institutions to plan continued care and to coordinate interventions related to finances, housing, family discord, and other psychosocial problems.
- Provides education to patients/families regarding Advance Directives for healthcare decision-making and assists with completion of these documents as appropriate.
- Other duties as assigned.
The Requirements:
- Social Worker licensure in the state of Wisconsin is required.
- Bachelor’s Degree in Social Work required; Master’s Degree in Social Work preferred.
- Demonstrated knowledge of social work principles and methods, and the ability to apply these in a healthcare setting is required.
- Must have demonstrated decision making, team building, time management, customer service, and conflict resolution skills.
- Excellent communication and human relations skills necessary.
- Computer skills to include MS Office and electronic medical record software needed.
- Prior experience providing social work services in a healthcare environment preferred.
