Key Responsibilities

- Complete Start of Care (SOC), Resumption of Care (ROC), Recertifications, and routine visits
- Administer medications and treatments per physician orders
- Assess patient conditions and report changes to the supervising RN or provider
- Educate patients and caregivers on care plans, medications, and safety
- Maintain accurate and timely clinical documentation
- Coordinate care with interdisciplinary team members
- Ensure compliance with agency policies, state regulations, and clinical standards
Systems & Documentation
- Required experience with OASIS
- Required experience with HCHB (Homecare Homebase)
Orientation Requirement
- Must attend an in-person orientation session with the Talent Team at the Elkhart branch
- Orientation is held on Mondays or Wednesdays
Qualifications
- Active Indiana LPN license (required)
- Home health experience preferred
- Proficiency with OASIS and HCHB required
- Valid driver’s license and reliable transportation
- Strong clinical, documentation, and communication skills
