Job Description:
The RN Navigator provides, supports, and coordinates a comprehensive rehab care continuum at UofL Health - Frazier Rehab. The Rehab Nurse Navigator is an integral part of the rehab team for patients prior to inpatient admissions and beyond outpatient rehab discharge. The Rehab Nurse Navigator provides patient care, education, documentation, and facilitates communication amongst patients, support systems, UofL Health team members, and physicians. The Rehab Nurse Navigator tracks performance improvement measures for specialty populations including patient experience initiatives and quality care metrics and demonstrates a commitment to improving the rehab services provided.
A focus of the Rehab Nurse Navigator is to promote patient and staff education by providing group education opportunities and competency training sessions. A Rehab Nurse Navigator will facilitate the development, coordination, and implementation of a patient’s plan of care. It is the responsibility of the Rehab Nurse Navigator to promote rehab staff development and competency completion in the delivery of care to patients with specific diagnoses requiring rehabilitation and serves as a key liaison with clinical educators. The Rehab Nurse Navigator ensures appropriate equipment, supplies, and proper training is available to support the clinical team and the specialty population. The Rehab Nurse Navigator facilitates plan of care management of for patients throughout the continuum of care and collaborates with other members of the care team as needed to optimize patient participation and outcomes.
Essential Functions:
- Conducts patient/family/caregiver education as needed across the care continuum, evaluates learning needs and sets objectives based on established standards; coordinates training opportunities and establishes competencies for patients and support systems
- Coordinates and participates in the patient plan of care from acute care to inpatient rehab to outpatient rehab to community re-integration with an emphasis on education, medical management, and prevention of secondary complications; provides a direct communication link between patient, physician, therapy team, and nursing team
- Develops and tracks performance improvement measures in collaboration with director, physician, nursing, and rehab team
- Monitors patient progress on an ongoing basis for early identification of changes in physical, cognitive, or psychosocial presentation requiring immediate intervention; collaborates with appropriate team members to ensure implementation of care plan to promote patient success
- Provides evidence-based education and training to UofL Health team members, physicians, students, and stakeholders as needed to improve quality care outcomes, patient safety and satisfaction outcomes, and increase community awareness of effective rehabilitative practice
Other Functions:
- Assists in delivering high quality healthcare services in a professional, compassionate, and courteous manner while respecting the dignity and individuality of each person who comes in contact with the organization
- Attends continuing education pertinent to clinical areas to maintain knowledge and skills in providing care to the designated specialty population
- Collaborates with other departments and outside agencies to meet the identified needs of the patients and the organization
- Facilitates community engagement opportunities to promote awareness and identify resources available
- Strategizes operational improvements to reduce healthcare costs through prevention of secondary complications related to spinal cord injury
- Develops and updates Departmental Policies and Procedures Manuals in accordance with JCAHO, CARF, State, Federal and other regulatory agency criteria, as needed
- Maintains compliance with all company policies, procedures and standards of conduct
- Complies with HIPAA privacy and security requirements to maintain confidentiality at all times
- Performs other duties as assigned
Education:
- Associate’s Degree in Nursing (ADN) (required)
- Bachelor of Science in Nursing (BSN) (preferred)
Experience:
- Two (2) years of experience in rehab nursing (required)
- More than two (2) years in rehab nursing and experience with the designated specialty population (preferred)
Licensure:
- Active Kentucky Registered Nurse License or compact license with privileges to work in Kentucky
Certification:
- Basic Life Support (BLS) accredited by the American Heart Association (AHA) (required)
- PALS certification (required for the Pediatric Specialty Program)
- Certified Registered Rehabilitation Nurse – CRRN (preferred)
- Program-specific certifications (i.e., CPN, CBIS) (preferred)
