INTRODUCTION
Sheppard Pratt is searching for a Registered Nurse to serve as Assistant Director for its Health Home program split between Gaithersburg and Lanham, MD.
The Health Home targets populations who are having their behavioral health needs met at Sheppard Pratt’s Day Program, Assertive Community Treatment (ACT) Program, or Residential Rehabilitation Program. The team provides an extra layer of care, providing an integrated approach for people who are living with both mental and physical illness.

ROLE AND RESPONSIBILITIES
You will coordinate somatic healthcare in tandem with mental illness, emotional disturbance and/or substance use disorder treatment. You will identify the gaps and barriers between behavioral and physical healthcare to develop wellness and prevention initiatives that ensure all care needs are met.
Once assigned a patient, you will ensure the implementation of their treatment plan in partnership with program leadership and staff, the individual, and their support system. You will be asked to:
- Monitor treatment plan goals.
- Coordinate access to medical providers and hospitals, including admission and discharge.
- Ensure patients complete all required assessments, screenings, and immunizations.
In an effort to continually improve patient care, the Assistant Director will also:
- Keep abreast of current trends and developments in the field.
- Participate in the evaluation of program effectiveness and ensure continuous improvements in the quality of services.
- Assist in training of Health Home staff, specifically nurses.
- Identify quality improvement opportunities and population-level care management.
- Develop and maintain partnership with external entities (hospitals, medical providers, and community organizations) that play a role in treatment plans.

- Must be a Registered Nurse in Maryland at time of application.
- Must have 1-3 years of nursing experience.
