The Integrated Mental Health Therapist (IMHT) works within a multidisciplinary team embedded in the primary care clinic and utilizes the Collaborative Care model of care. This team includes the IMHT, Psychiatric Consultant, & Primary Care Provider to provide comprehensive behavioral health services within the primary care setting. IMHT will provide evidence-based interventions, in partnership with the treatment team, to promote symptom reduction and optimal patient outcomes.
Responsibilities:
- Assesses patient’s symptoms and create customized treatment plan based on individual patient needs
- Screens patients for common behavioral health disorders using PHQ9, GAD7, Columbia Suicide Risk Assessment, or other tools as required
- Monitors patients for changes in clinical symptoms and treatment side effects/complications, with a goal of reduction in symptoms
- Supports psychotropic medication management prescribed by PCPs, focusing on treatment adherence, side effects and other complications, and treatment effectiveness
- Tracks patient follow-up and clinical outcomes using a registry, and uses the system to identify and re-engage patients
- Facilitates care plan changes for patients who are not improving, including changes in medications or psychosocial treatments or appropriate referrals for additional services
- Manages behavioral health crisis as they arise in a primary care setting
- Provides flexible behavioral health services to patients on and off current caseload, both through scheduled appointments and immediate interventions via warm handoffs during medical visits with primary care providers

Position Summary
The Licensed Integrated Mental Health Therapist-BHN is responsible for independently providing assessments, treatment/care planning and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care and chronic disease management patients. The incumbent uses a variety of theoretical treatment models and interventions and excellent communication skills to work within a primary care/chronic disease management team setting with limited supervision; documents treatment/communication in keeping with department standards and keeps current on community resources to effectively provide linkage and advocacy to fulfill client’s needs.
Requirements
- Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or related field
- A Mental Health Professional as defined within HB 1724 one of these higher credentials are held: Licensed Marriage & Family Therapist (LMFT), Licensed Mental Health Counselor (LMHC), SW Independent Clinical Lic (LICSW)
- Minimum two (2) years of experience in a behavioral health setting

Pay and Benefit Expectations
We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $38.38 - $55.23 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
