INTRODUCTION
NaphCare is hiring a Mental Health Discharge Planner to join our team of medical professionals in the Wayne County Jail in Detroit, Michigan. Pay rates range from $22.80 - $24.00 per hour plus shift differentials where applicable! Pay rates range per hour based on employment status, site needs, licensure, and experience. As the Correctional Mental Health Discharge Planner, you will work alongside correctional officers and other medical professionals to ensure high-quality care is provided to the individuals in residence. This is a unique position where you can make a difference to this diverse population of patients with complex mental health and medical needs.
Responsibilities
The Mental Health Discharge Planner is responsible for coordinating the transition of patients from the mental health unit back into the community. Duties and responsibilities may include and are not limited to:
- Conducting comprehensive assessments of patients' needs, including psychosocial, medical, and functional status
- Collaborating with patients, families, security staff, and the interdisciplinary treatment team (physicians, nurses, social workers, etc.) to develop individualized discharge plans that address identified needs, such as medication management, therapy, housing, and vocational support
- Coordinating and facilitating referrals to appropriate community resources, including mental health services, substance abuse treatment, housing, and financial assistance
- Connecting patients with support groups and other community-based organizations
- Ensuring all necessary paperwork is completed, including consent forms and referral authorizations and maintaining accurate and timely documentation of all discharge planning activities in patient charts
- Monitoring compliance with site-specific and company policies and procedures, taking appropriate corrective action, and ensuring compliance with ACA, NCCHC standards
BASIC QUALIFICATIONS
Education / Licenses / Certifications
- Bachelor's degree in counseling, social work, psychology or related field
- Possess an active CPR certification
Experience / Requirements / Qualifications / Skills
- Must have knowledge and experience in the development of mental health discharge plans and coordination with community service providers
- Must be able to remain calm under pressure while meeting with patients to discuss placement options, organize records for transition, and provide advice regarding community assistance organization, transportation options, etc.
- Effective communications skills are required for building patient rapport and relaying information to community resources, and other medical and mental health care professionals
- Strong organizational skills are needed to keep track of patient discharge plans
- Must have the ability to maintain confidentiality of all proprietary and/or confidential information and integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements
- Must pass the facility’s criminal background check and drug screening
PHYSICAL CAPABILITIES
Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift.
