JOB SUMMARY
Scheduled as needed; REQUIRES: Bachelors Degree and one year related experience; previous addiction treatment experience strongly preferred.
JOB DUTIES
Functions as primary care manager at all levels of counseling care as needed, providing chemical dependency services to patients and their families in accordance with established policies and professional standards.
- Responsible for carrying out all relevant charting requirements.
- Facilitating group therapy, individual and family therapy.
- Presents educational lectures to patients and family members.
- Coordinates case management responsibilities with outside agencies and coordinate discharge planning.
- Observes all applicable facility policies and procedures.
- Coordinates treatment planning process through the multidisciplinary team.
- Participates in department quality improvement activities.
