INTRODUCTION
A mental health and drug and alcohol recovery agency that provides treatment services is seeking 2 Social Workers/Counselors with experience in providing case management and alcohol and drug counseling to a wide range of individuals with substance abuse and mental health disorders in Cleveland, Ohio. These individuals will provide assessments, diagnosis, treatment planning, and individual and group counseling, in this Medication Assisted Treatment program, while meeting all standards of practice as required by the regulatory bodies.
ROLE AND RESPONSIBILITIES
- Conduct comprehensive in-person needs assessment on an ongoing basis and as required. Assessment includes obtaining the consumer’s health history through consumer/caregiver reporting, data collection, medical records, and collaboration with physicians and related health care professionals. Assessment addresses the consumer’s functional, psychosocial, socio-cultural, and resource/financial status in relationship to their strengths, challenges, opportunities, and formal/informal support systems.
- Establish a care plan to meet the consumer’s health care needs in the community. Care planning includes direct intervention by the care manager and direct service provision from community resources and from the contracted provider network.
- Implement and coordinate the delivery of services through the authorization of contracted services, through referrals to community agencies and through the mobilization of consumer’s informal support.
- Evaluate and monitor the appropriateness of the care plan in meeting consumer needs based on consumer outcomes and make necessary adjustments to the plan to assure that goals are met.
- Educate the consumer on issues of health promotion, illness prevention, crisis management, general well-being, and in maneuvering within the health care system.
- Participate in training, supervisory meetings, in-services and continuing education opportunities.
BASIC QUALIFICATIONS
- Bachelor's Degree with extensive counseling experience.
- Master's degree preferred.
- MUST have a valid and current Ohio licensure as an LSW, LPC, LCDC or LICDC LISW
- 0-3 years of Counseling and case management experience.
- Experience in providing treatment to individuals with addictions and mental health illnesses a plus.
- Experience working in a variety of treatment settings including residential or outpatient settings.
- Experience conducting individual, group and case management services. Knowledge of crisis intervention, multicultural and socio-economic issues. Basic advocacy skills. Ability to utilize analysis, experience, and judgment to make sound decisions. Ability to develop trust and rapport with clients and maintain confidentiality.
- Familiarity with community support groups and resources.
- Excellent written and verbal communication skills.
- Must be compassionate.
