INTRODUCTION
Carle Health is seeking a Family Medicine Advanced Practice Provider to join our team at our Primary Care team in Mahomet, Illinois.
OPPORTUNITY DETAILS:
- Typical schedule M-F / 8:00a-5:00pm
- Family Medicine APP works with a collaborative team including the physician, APP, nurse and certified medical assistant to provide patient care in an ambulatory setting.
- Possibility for in office procedures
- Broad scope of patients including mostly adults, roughly 20% peds
EDUCATION REQUIREMENTS:
Master's degree in Nursing or Physician Assistant.
Two years of clinical practice is preferred.
LICENSURE/CERTIFICATION REQUIREMENTS:
Current Illinois registered nurse license and advanced practice nurse/physician assistant licensure.

PROVIDER SALARY RANGE:
The compensation range for this position is $56.37 - $73.29/hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
