Certified Community Behavioral Health Clinic (CCBHC) Core Services includes:
- Crisis mental health services including 24-hour mobile crisis teams, emergency crisis intervention, and crisis stabilization
- Screening, assessment, and diagnosis including risk assessment
- Patient-Centered treatment planning or similar processes, including risk assessment and crisis planning
- Outpatient mental health and substance use services
- Outpatient clinic primary care screening and monitoring of key health indicators and health risk
- Targeted case management
- Psychiatric rehabilitation services
- Peer support, counselor services, and family support services
- Intensive community-based mental health care for members of the armed forces & veterans
SUMMARY: The CCBHC Psychiatrist will work with a team of professional social workers, clinicians, case managers, and peers to ensure integrated care for adults experiencing a serious mental illness and/or substance abuse disorder, and a chronic health condition. As a member of the client care team, the Psychiatrist will establish a therapeutic relationship with the client to facilitate the assessment, diagnosis, prescribing of medication, monitoring client progress, and adjusting treatment to better stabilize MH, SU, and health needs of clients living in the community for integrated and person-centered care. To be effective, the Psychiatrist will have the demonstrated ability to balance independent clinical decision making with appropriate consultation with other providers and work with the CCBHC multi-disciplinary team to engage, establish, and achieve appropriate treatment goals.
JOB DESCRIPTION:
- Perform evaluation and assessment of clients requesting substance abuse treatment who may have a mental health disorder and/or chronic health condition/s
- Perform medication evaluation to establish and prescribe medication treatment as part of an integrated treatment approach
- Perform ongoing pharmacologic treatment and monitoring of the client’s experience with the medication and barriers to medication adherence
- Participate in incident review as part of a multidisciplinary team, as appropriate and needed
- Actively participate in treatment planning for an integrated person-centered approach and perform updates as required by program policy and standards
- Participate in case conferences with the multidisciplinary team, as needed
- Perform accurate diagnosis of clients and collaboratively work with CCBHC team to improve client’s progress toward treatment goals
- Monitor client to check progress and adjust the treatment plan as needed
- Thoroughly document each client's diagnosis, symptoms, medication, treatment effectiveness, etc.
- Provide patients with emotional support and strategies to help manage their disorder
- Research new developments in psychiatry and mental health. Incorporate evidence-based practices including cognitive behavioral therapy and self-management
- Train staff, as needed, on management of substance use related health problems and specifically on symptoms of withdrawal and opioid use
- Provide consultation on MAT to clinical staff across sites
- Notify Infection Control Coordinator immediately of suspected or confirmed communicable diseases, which can be transmitted through routine shelter contact
- Ensure psychiatric evaluations and medication management documentation meet all stakeholder, payer, and regulatory expectations and include clinically appropriate diagnoses and recommendations
- Utilize evidence-based practices as appropriate for the individual client and program philosophy
- Prescribe buprenorphine-naloxone / buprenorphine to patients who are enrolled in the MAT program and who are in compliance with treatment contract
Experience:
- Extensive knowledge of the DSM
- Expertise in diagnosis and treatment of substance use disorders
- Extensive experience with MAT
- Knowledge of the intersection of mental health, substance misuse, and chronic health conditions
- Experience with adults with mental health and/or substance abuse needs
- Experience with performing evaluations, prescribing, and medication monitoring
