How you'll contribute as an employee of CTRH you will join a team that embodies a culture of teamwork and effective communication, essential for the delivery of patient care. You will work with a team committed to ensure patients receive optimal medical attention and support. As an LPN you will provide direct patient care under the supervision of a Registered Nurse, delivering treatments, assisting in assessments, and supporting interdisciplinary care planning in accordance with the patient’s needs.
- Administer medications safely, accurately, and based on patient age and clinical condition.
- Minimize potential for medication errors by following proper medication administration procedures.
- Assist the RN with initial assessments and patient reassessments per policy.
- Identify and implement appropriate rehabilitation nursing interventions.
- Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
- Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
- Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
- Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
- Develop and implement individualized teaching plans based on patient and family learning needs.
- Perform other duties as assigned.

Qualifications and requirements:
Education: Graduation from an accredited Licensed Practical/Vocational Nurse program.
Licensure: Current state licensure as a Licensed Practical/Vocational Nurse.
Certifications: CPR/BCLS certification. IV Certification highly preferred or must be obtained within 6 months
Experience: Minimum of six months’ Medical/Surgical experience in an acute care setting required.
Excellent oral and written communication and interpersonal skills
