Summary
Job Description:
Part Time 24 weekly hours, day shift (7A-7P)
Job Summary
- Responsible for coordinating and assessing all patients referred to the Access Center from the Emergency Department and Main Hospital.
- Coordinates the approval and level of care determination in cooperation with the psychiatrist, patient/family and insurance verification.
- Coordinates and facilitates outside referrals and placement if needed.

Ensuring that the work performed generally aligns with identified department and organizational goals, including Baptist Health’s Strategic Pillars.

Participates in the formulation of a treatment plan, implementation and evaluation.
- Implement therapeutic, evidenced based group therapy and psychoeducational classes to meet the needs of the patients.
- Provide brief therapeutic counseling.
- Assist patients with paperwork needed for FMLA and Short-term Disability in order to be away from work while in treatment.
- Develops and maintains professional relationships with treatment team members, hospital and medical staff and community partners.
Minimum Education, Experience, Training and Licensures Required
- Master's Degree required.
- 2 years of job-related experience required.
- Experience working in a hospital setting; knowledge of substance use disorder treatment; experience in group therapy preferred.
- Active Kentucky License: LCSW, LPCC, or LMFT.
- Incumbent may also be an RN: Active KY RN license required. 2 years psychiatric nursing required. Must obtain ANCC certification in psychiatric/mental health nursing within one year.
