Part Time 24 weekly hours, day shift (7A-7P)

Job Summary

• Responsible for coordinating and assessing all patients referred to the Access Center from the Emergency Department and Main Hospital.
• Coordinates the approval and level of care determination in cooperation with the psychiatrist, patient/family and insurance verification.
• Coordinates and facilitates outside referrals and placement if needed.

• Ensuring that the work performed generally aligns with identified department and organizational goals, including Baptist Health’s Strategic Pillars.

• Participates in the formulation of a treatment plan, implementation and evaluation.

• Implement therapeutic, evidenced based group therapy and psychoeducational classes to meet the needs of the patients.
• Provide brief therapeutic counseling.
• Assist patients with paperwork needed for FMLA and Short-term Disability in order to be away from work while in treatment.
• Develops and maintains professional relationships with treatment team members, hospital and medical staff and community partners.

Minimum Education, Experience, Training and Licensures Required

• Master's Degree required.
• 2 years of job-related experience required.
• Experience working in a hospital setting; knowledge of substance use disorder treatment; experience in group therapy preferred.
• Active Kentucky License: LCSW, LPCC, or LMFT.
• Incumbent may also be an RN: Active KY RN license required. 2 years psychiatric nursing required. Must obtain ANCC certification in psychiatric/mental health nursing within one year.
