Develop and manage treatment plans for acute and chronic cardiac conditions
Provide patient education on heart health, disease prevention, and medication management
Collaborate with primary care providers and care teams to ensure coordinated care
Document all encounters accurately in the electronic health record (EHR)Participate in quality improvement and clinical meetings as requested
Qualifications:
Medical degree (MD or DO) from an accredited institution
Board certified or board eligible in Cardiology
Current New York State medical license (or eligibility for licensure)
Experience in outpatient or community health settings preferred
Strong communication and patient engagement skills
