With deep roots in Western Massachusetts and a growing presence in communities throughout the state, we are currently seeking a Care Coordinator II to join our Adult Community Clinical Supports (ACCS) team and help advance our mission of empowering people to live healthier, more fulfilling lives.

WHAT YOU WILL DO:
- Provide direct interventions with individuals served to complete comprehensive assessments, treatment plans, and clinical interventions
- Provide modeling and clinical oversight for outreach and GLE direct care staff
- Conduct and oversee clinical services set forth by the Integrated Team, performing functions reserved for the Licensed Practitioner of Healing Arts (LPHA) as specified by contract
- Oversee services provided to all individuals served by ACCS
- Ensure the program adheres to clinical regulations and funding source requirements
- Produce documentation that is clear, legible, and meets DMH and ACCS standards
- Develop treatment plans consistent with evidence-based and best practices
BASIC QUALIFICATIONS:
- Master's Degree in a mental health-related field required
- Clinical licensure required (e.g., LCSW, LICSW, LMHC)
- Minimum of 2 years' experience in the human service field
- Committed to the empowerment and growth of individuals served, human rights, and community integration
- Knowledgeable in the interface between mental health and medical conditions, substance abuse, care management, recovery principles, evidence-based practices, and trauma-informed treatment
- Strong verbal and written communication skills
