Onsite @ Greater Baltimore Medical Center (GBMC) | 6701 Charles Street, Towson, MD
STATUS: Full Time, 40 Hours/week | HOURS: M - F, 8:30am - 5:00pm
Onsite @ Johns Hopkins Hospital (JHH) | 600 N. Wolfe Street, Baltimore, MD
HOURS: Every Other Weekend: S & S 8:30am - 4pm
JOB SUMMARY:
The Home Care Coordinator Assistant supports the home care coordination team by helping arrange services and equipment needed for patients transitioning from hospital care to home care. This office-based position located at GBMC and JHH plays an important role in connecting patients with home health services, medical equipment, and community resources. In this role, you will also assist with administrative tasks that ensure referrals, documentation, and coordination activities run smoothly, helping to support seamless care transitions for patients.

MINIMUM QUALIFICATIONS:
Education:
- High school diploma or equivalent required, some college preferred
Work Experience:
- One-year data entry experience
- One-year customer service experience in a health care setting
- Previous scheduling experience preferred

- Strong interpersonal, organizational, and communication skills (both written and verbal)
- The ability to multitask and manage competing priorities in a fast-paced environment
- Confidence working independently in situations that may require initiative and problem-solving
- A commitment to delivering excellent customer service to patients, hospital staff, agencies, and partners
- The ability to travel between multiple inpatient and outpatient hospital locations
- Strong attention to detail with the ability to enter and manage data accurately and efficiently
- The ability to collaborate effectively as part of a team
- Flexibility and adaptability in a changing environment

Salary Range: Minimum $16.34/hour - Maximum $26.94/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
